TERMS AND CONDITIONS
Benefits are released from Fit Advantage Solutions LLC of Michigan, (FAS of MI), based on your participation in the Fit Ticket™ Community Network. You must read and agree with and accept the terms and conditions below:
· I have provided true and accurate information when setting up my user account. I maintain no more than one user account and my account is for personal use. I am at least 16 years old. I agree to cooperate in any investigation, due to identity or any information I have willing and unwillingly provided to the FAS of MI, or sub company FTC Community Network, (Fit Ticket).
· I acknowledge and fully understand FAS of MI is an independent contractor to Fit Ticket AKA, FTC Network™. FAS of MI, is a service provider for consumers, advertisers, and fitness service organizations.
· Under no circumstances does FAS of MI, provide any warrantees to any goods or services offered by advertisers, contractors or likewise. Any problem or complaint that I have regarding goods or services in respect to this contract will remain addressed directly with advertiser/contract provider.
· I acknowledge that any benefit I receive from the FAS of MI, is only from partaking in advertisements/promotions/or using a respective service based on advertisers information.
Fit Centz “reward value” is based on a multiplication factor of 2 times today’s monetary value for services and goods. FAS of MI is not responsible for service exchanges, service or liability that could or may arise within a service transaction, or within advertising of provider advertisers.
Advertiser/ Grantor/Service Provider is responsible for personal liability, terms, or contracts based on facility requirements.
· I am legally capable to partake in any or all physical exercise. I have been cleared of any condition(s) with a legal doctor, pertaining to my health, my physical condition, and would not hold any participating party or FAS of MI, or likewise affiliated contactor responsible. I am responsible for any or all services I could partake in, through a contact/provider/advertiser at affiliate locations.
· It is my responsibility to maintain correct profile information, and remain in a condition to exercise, or remain physically active. I also agree to have regular exams or checkups, prior to becoming physically active. All transactions are final, and Fit Ticket rewards/Fit Centz are final payment for redeeming services.
· No refunds are allowed or required. Contracts with service providers, advertisers, or contractors will become disputes with parties in questions, excluding FAS of MI, FTC Network, or Fit Ticket.
· I have, with the FAS of MI, provided correct profile information.
· Resolutions to problems that may arise will be in the form of rewards.
· I agree that I have not violated these Terms and Conditions or any other agreement I have entered into with the FAS of MI, or affiliates. I have not cheated this or any system in any way, which to gain additional cash, reward or compensation. I acknowledge that payments to me will be in rewards, recorded in profile, based on incentives and promotional gains, and activity. At any time reward (Fit Centz) can be reversed on my account if I am found to violate these terms in any way
· It is my responsibility to determine what, if any, taxes could apply to the payments I have received, as rewards. Based on my personal circumstances it is my responsibility to report and pay the correct tax to the appropriate tax authorities. I acknowledge that the FAS of MI and affiliates may report my benefit compensation information to the respective authorities if required.
· I ACKNOWLEDGE THAT IN NO EVENT SHALL, FAS of MI, Fit Ticket, FTC Network, ITS OFFICERS, DIRECTORS, AGENTS, JOINT VENTURERS, EMPLOYEES AND SUPPLIERS PROVIDERS be LIABLE FOR INCIDENTAL OR CONSEQUENTIAL DAMAGES ARISING OUT OF OR IN CONNECTION WITH THE FIT TICKET (FTC NETWORK) SERVICE OR ANY GOODS OR SERVICES PURCHASED FROM AN ADVERTISER.
· I ACKNOWLEDGE THAT FIT TICKET (FTC NETWORK) IS PROVIDED “AS IS” AND WITHOUT ANY REPRESENTATION OF WARRANTY, WHETHER EXPRESS, IMPLIED OR STATUTORY. I FURTHER RECOGNIZE THAT FIT TICKET COMMUNITY NETWORK, FAS OF MI, or FIT TICKET, ITS OFFICERS, DIRECTORS, AGENTS, JOINT VENTURERS, EMPLOYEES AND SUPPLIERS PROVIDERS, SPECIFICALLY DISCLAIM ANY IMPLIED WARRANTIES OF TITLE, MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE AND NON-INFRINGEMENT.
